Participant Agreement and Assumption of Risk for Horseback Riding

The following participant agreement is to be used only for large groups or families where one person (Group Leader) is responsible for several other people. It is most often used by Youth, Church and Scouting Groups participating in Whitewater Rafting and Ropes Course Activities.

Please note when using the multiple activities waiver that all participants must be listed on Page 1, with the Group Leader's Information on the top of page 1. You may make copies of this form for additional participants.

Participation requires a separate page 3 for each and every participant, signed by Parent or Guardian if under 18
